婚姻注册申请表格

婚姻注册申请手续：

两位新人需亲自到政府注册局申请，并携带以下证件:
Both Groom & Bride should personally apply to JPN and bring along the following documents:

★ 双方身份证正本与副本
Original and photocopy of Groom & Bride´s I/C

★ 未满二十一岁者,男女双方需携带报生纸正本与副本
Original and Photocopy of Groom & Bride´s Birth Cert for applicant aged below 21 years old

★ 双方护照型蓝底片各一张( 32 x 30mm, 人头要大，近照)
Groom & Bride´s recent passport size photograph (Size: 32 x 38mm)

★ 未满二十一岁者,父亲必须陪同到政府注册局申请，同时需成为其证婚人。
The father of applicant aged below 21 years old must accompany the applicant to JPN to sign
form JPN.KC01B and must be the Witness of the Marriage.

★ 离婚者: 离婚证书正本与副本
For divorcee, please bring the original and photocopy of The Divorce Certificate/Sijil
Perakuan for verification.

★ 鳏夫或寡妇者，需携带其配偶之死亡证书副本(Sijil Mati)
Widow/Widower to bring along a copy of Death Certificate of spouse.


资料经由政府注册局批准后，方可前来福灵宫安排注册日期。
After verification by JPN, Groom and Bride are requested to make arrangement for Wedding
Ceremony date at Persatuan Agama Buddha Hock Lin.

★ 呈交本宫时需携带，两位证婚人身份证副本
Applicant must submit the documents verified by JPN together with photocopy IC of
2 Witnessess to Persatuan Penganut Agama Buddha Hock Lin.
