1. 男女雙方年齡已足21歲，馬來西亞公民未婚者，且是佛教徒。填寫表格，須備下列文件：
（A） 男女雙方正版身份證，並複印各1份（即：上正面，下反面；不可以画线或写上任何文字）
（B） 男女雙方正版報生紙。（不足21歲，複印2張）
（C） 男女雙方PASSPORT SIZE （32mm x 38mm）正面 藍底或白底 近照各1張。
（D） 複印2位證婚人身份證各1張（即：上正面，下反面，不可以畫線或寫上任何文字。）
（E） 婚姻註冊申請表格BORANG JPN.KC02 必須完整填妥。（本廟備有此表格）
（F） 註冊表格 宣誓 後，, 必須到政府婚姻註冊局还印花稅，交回本廟張貼佈告處21天。
（G） 若沒人反對，待21天後至6個月內即可在任何星期六，星期日 或特別開放日註冊。
2. 男/女年齡由18歲至21歲
年齡不足21歲者：其父親須 親自 到政府婚姻註冊局簽署1張同意書（BORANG JPN. KC01B）。
3. 女性年齡由16歲至18歲
其父親须 親自 到政府婚姻註冊局，簽署1張 同意書（BORANG JPN. KCO1D），待批准後，1個月內即要註冊。
4. 離婚者：須呈上國民登記局JPN 婚姻註冊部發出的離婚證實文件。
5. 喪偶者：須呈上正版 配偶死亡證書。
6. 與外國人婚嫁：請到政府婚姻註冊局辦理手續。
MARRIAGE REGISTRATION GUIDELINE
1. Both applicants must achieve age of 21, a Malaysian citizen, not married; either he or she must be a Buddhist. Please bring along the below listed documents which will be submitted to The Registrar of Marriage for verification:
(A) To bring along original Identity Card (IC), IC photocopy of both sides, front on page top, back on page bottom. (Leave it blank and do not cross.)
(B) To bring along original Birth Certificate. (Under age 21 come with 2 pieces of 2 photocopies)
(C) To bring along a piece of passport size (32mm x 38mm) photo in blue or white background.
(D) Both witnesses to bring along IC photocopy of both sides, front on page top, back on page bottom. (Leave it blank and do not cross.)
(E) Application form of BORANG JPN.KC02 must be filled up completely. (Application form can be obtained from office)
(F) After BORANG JPN.KC02 to be vowed by authorized body, i.e.: Persuruhanjaya Sumpah, and stamp duty to be paid, it must then to be published on selected notice board for at least 21 days.
(G) If there is no opposition toward application, after 21 days, a date to be nominated for conducting ceremony of marriage registration in temple, but must not later than 6 months.
2. Applicant who is within age of 18 to 21, his or her father must go to The Registrar of Marriage, filling up and sign BORANG JPN. KC01B.
3. Female applicant in the age of 16 to 18, her father must go to The Registrar of Marriage to fill up and sign BORANG JPN. KC01D and the registration must be made within 1 month after approval.
4. For divorcee, original copy of Certificate of Divorce from JPN must be presented upon filling up application form.
5. Original Certificate of Death must be presented for those whose spouses have passed away.
6. Application which involve foreigner is not applicable in our office, and they shall proceed to The Registrar of Marriage.
