A Loving Connection between Children with Disabilities and Resources They Need

Temporary Financial Assistance Available

To be eligible for this program, you must follow the criteria

• Have a child with physical or mental disability diagonised within the last 6 months (the diagonise can be prenatal or at any age)
• Be at below the state of New Mexico poverty line

To apply, please do the following

1. Pick up an application from ChildLink (address bellow)
2. Assemble the following papers
   1. Your completed application
   2. Doctor's records of your child diagosis
   3. Tax records or New Mexico Social Service certificate of your income level
   4. Your lease, mortage, or medical bills, depending on which you need help with
3. Make an appointment with a ChildLink volunteer, available three times:
   1. Ida: MW 10:30 a.m. to 3:30 p.m.
   2. Juan: TR 9:00 a.m. to noon
   3. Chris: F 10:30 a.m. to 3:30 p.m.
